Impact Health PC is a Nurse Practitioner Group contracted with Skilled Nursing Facilities and Rehab Centers throughout the Northeast that strives to provide the best in medical support and consultation. Our core mission puts the patient first, always. We are hyper-focused on developing treatment plans in collaboration with facility staff, physicians, specialists and most importantly, the patient.

Practitioners are allowed to create a flexible schedule Monday through Friday that works best for them and their assigned building. Our practitioners are vital members of the Medical Staff and considered primary care leaders, partnering with nurse managers, administration, and the medical directors to treat in place, successfully return short-term rehab patients home and participate in facility initiatives that achieve optimal results.

Qualifications:

Family or Geriatric APRN License

2+ Years as an APRN

Experience in Skilled Nursing Facilities, Hospitalist, Emergency Dept, SICU/ICU

Proficient with tech and EHR's

Responsibilities:

Provide person-centered care using best practices

Effectively communicate and collaborate with Medical Staff and nursing

Document within 24 hours of encounter

Adhere to state regulations within SNF setting

Provide assessment, diagnostic, and treatment services

Provide counsel and education to patients, their families and staff
