Description:

Impact Health is a nurse practitioner group serving skilled nursing facilities across the state with both medical and psychiatric care. Impact Mental Health has full-time opening for Connecticut based APRN Clinical Lead. The Clinical Lead would be supported by our team of APRNs, LCSWs, psychologists, and MDs. While providing a strong team atmosphere, Impact allows its providers to practice with autonomy and independence. Impact services multiple nursing facilities throughout the state and is growing! We provide a full-benefit package with a competitive salary and room to grow. Providers are allowed to create a schedule that works best for them and their assigned buildings within typical business hours, 8am - 5pm.

Job Responsibilities:

Provide support and clinical consultation to Psychiatric Nurse Practitioners

Provide training and education to onboard Psychiatric Nurse Practitioners

Assist with maintaining compliance with SNF regulations related to psychiatric providers

Provide support with clinical oversight

Provide person-centered care using best practices

Work within a team of social workers, therapists, nurses, and physicians to coordinate care within Impact and the skilled nursing facility

Collaborate with medical and psych team as needed

Submit required documentation within 24 hours of encounter

Provide assessment, diagnostic, and treatment services as needed

Experience:

Experience in skilled nursing facilities, hospitals, residential care, geriatric care, and psychiatric outpatient preferred

Qualifications:

Family, Geriatric, or Psychiatric Nurse Practitioner with active license in Connecticut

2+ years clinical experience. Nursing home experience preferred.
